The film begins with two teenagers on their motorcycles driving away from pumpkinhead. Pumpkinhead then kills one of the teenagers. The surviving teenager named Dallas goes to the man who conjured pumpkinhead and begs him to call the demon off. Pumpkinhead then smashes the window and claws Dallas in the chest. Dallas then kills the demon by killing the summoner. Ed Harley then appears telling Dallas, Pumpkinead will return and there will be no place to hide. Five years later the family of the Hatfields and McCoys have started a feud because of a car. The Hatfields then trash the McCoy wedding. Jody Hatfield sneaks out to see her true love, Ricky McCoy. Ricky brings his sister, Sarah, to look out for him and Jody. The two then start to make-out. Jodys brothers then find and kill Sarah by accident. Jodys brothers then try to kill Ricky. Ricky then finds his dead sister in the woods. Ricky then goes to Haggis for help and the two invoke Pumpkinhead to kill the Hatfields. The film continues to effectively ignore the events portrayed in the Pumpkinhead II: Blood Wings sequel, with Lance Henriksen returning as the ghost of Ed Harley, his character from the first film. The film is criticized as the best of the sequels and is the final chapter in the Pumpkinhead series
